This form can be used by medical professionals to add their medical clinic to the TruckSafe Member Access portal.

TruckSafe members in your area will know that your doctors are familiar with carrying out Austroads Assessing Fitness to Drive (AFTD) commercial medicals, or if you offer other medical services for the transport industry.

Please note the following:

The Fitness for Duty certificate used in the medical assessment can only be signed by a general practitioner

The name of the person entered on this form will not be made publicly available

The email address entered on this form will not be made publicly available

This form is for the use of medical professionals only. It is NOT intended to be used by TruckSafe members to nominate a clinic

Once this form has been submitted we will contact the clinic to confirm they have been added to TruckSafe’s database.

A reminder that if you intend to conduct driver medicals it is a requirement of TruckSafe and the NHVR that an AFTD medical must be signed off by a general practitioner.
